Carrier Sekani Family Services (CSFS) is seeking a Community Health Nurse for the Vanderhoof area.

The Community Health Nurse [CHN] provides community health promotion services that promote the prevention of disease, maintenance of health and the overall health and well-being of individuals, families and the community.

The Community Health Nurse initiates treatment including health care management and therapeutic interventions in accordance with nursing statutory and regulatory standards, limits, and conditions, and employer policies and procedures.

Duties and Responsibilities

Delivers patient/client care in accordance with established standards, policies, procedures and the goals, objectives, guidelines and directions of Carrier Sekani Family Services by performing duties such as:

Performing nursing skills, such as delegated medical functions and crisis intervention, such as emergency assessment and care including arranging for escort and evacuation for further intervention as required, point of care testing, specimen acquisition- including venipuncture, and arranging referral where necessary;

Assessing, planning, implementing, documenting and evaluating the care needs and goals for individual patients/clients and their caregivers/families;

Processing and implementing physicians and or NP orders, identifying and seeking clarification as necessary;

Providing information to staff, including patient/client specific care instruction;
Reviewing patient/client care needs and treatment goals; planning, implementing and evaluating changes to the written Treatment Plan;

Typical practice areas include: Utilizing a variety of established health education, promotion and service interventions, such as Pre/Post-natal Care; palliative care, wound and skin care, medication management and administration, delegation of tasks to Community Health Workers, chronic disease management, Health Services for Community Living, continence management and gerontology.

Optimizing capacities for health, by modeling, counseling and teaching life skills for individual patients/clients and their caregivers/families;

Using epidemiological methods to investigate and follow-up on re portable communicable diseases;

Immunizing children and adults;

Conducting home and follow-up visits to monitor care being provided in a safe, efficient and effective manner and the achievement of identified service goals;

Promoting and facilitating patient/client independence;

Collaborating with the multidisciplinary team of health care professionals and health care services and providers to facilitate patient/client care;

Coordinating care provision with other programs/services.

2. Uses evidence and research and synthesizes nursing and health knowledge from a broad range of theories, models and frameworks to address the health status of the population as well as the factors that influence the delivery and use of nursing and health services.

3. Assigns care to other members of the health care team

Conducting home care intake assessments, case management, scheduling and supervision of resident care attendants;

4. Provides health education and promotion and treatment services for patients/clients, including individuals, families, groups and communities, by:

Teaching treatment procedures and care techniques and presenting health care information to lay people such as family caregivers through lectures, demonstrations and health forums;

Using a variety of modalities and applying techniques of social marketing and principals of teaching to targeted populations, and making presentations to special interest groups; and utilizes the community development process to facilitate and advance positive social change, including quality of life, wellness, and disease and illness prevention.

5. Maintains, protects and secures all patient/client records and supplementary related documents by monitoring access and maintaining appropriate disposition of the documents/information to protect confidentiality. Provides patient data to others as specified by CSFS policy.

6. As part of the Primary Care Cluster, participates in case conferences, treatment plan reviews, and staff meetings both by distance, using a variety of communication resources such as telephone, the internet, and video conference and in person.

7. Promotes good public relations and community awareness within the multidisciplinary team, and with others, including community resources such as local RCMP, ambulance crew, schools, government, and band councils.

8.Uses the MOIS electronic health record to maintain patient specific clinical information and ensures clerical and administrative processes are followed to document and collect required information. Collecting user fees, completing invoices, issuing receipts, processing forms, and entering data in the computer may also be required.

Provides written reports and submits statistical data regularly and as required.

9. Contributes to a safe and healthy working environment by observing and promoting universal precautions and infection control procedures; removing obvious hazards; reporting faulty equipment, accidents, injuries and near misses. Participates in the coordinated rural community disaster plan.

10. Demonstrates personal initiative in maintaining professional nursing knowledge and skills by participating in relevant educational activities as required by the employer.

11. Participates in fire prevention and disaster plans as required. Performs other related duties as required.

Qualifications and Educational Requirements

Baccalaureate Degree in Nursing and/or Registered Nurse;

Current practicing registration with the College of Registered Nurses’ of British Columbia (CRNBC)

Basic Life Support, Level “C” certificate

Acceptable experience in a clinical health setting or cross-cultural health promotion field;
Transportation of Dangerous Goods (TDG) course

Knowledge, Skills, and Abilities

Knowledge of First Nations Culture and traditions (specifically Carrier and Sekani)
Completion of PHSA Indigenous Cultural Competency Training is an asset.

Initial and on-going employment is subject to submission of a clear Criminal Record check. Criminal Record Check must be conducted every three years.

Must possess a valid driver’s license with access to a reliable vehicle.
Must possess BCCDC immunization certificate
